PH 02/26 — Port Hedland vessel inspection criteria

Effective 1 May 2026. Machinery physical inspections are carried out at anchorage only, or up to 30 days prior to arrival at the load or discharge port. The Port Hedland Vessel Vetting Form remains a Pilbara Ports controlled document completed by the vessel; this inspection does not replace it.

One or more trigger makes a machinery physical inspection mandatory. The Harbour Master may also direct an inspection under Trigger E.

A repeat finding is evidence the corrective action did not hold. It is read as a system issue, not a defect, and it carries weight in both the report and any vetting review.

Effectiveness of the safety management system

A.1206(34) turns on effectiveness, not on whether a procedure exists. Operational control is a check on the effectiveness of communication, interaction and familiarity of the crew, and clear grounds arise where the system is found not to be effectively implemented.

Areas marked not inspected are left out of the report entirely, so the document only shows what was actually examined. The report opens in Word for editing before it goes out. Photographs are resized as the report is built so the finished document stays under the 10 MB most mail servers accept. Building it needs no signal — the share sheet offers Mail, Signal, AirDrop and Files, and if you are offline it saves to the device for sending later.
